It is the parish church of the ancient village of Montepagano and was built at the end of the 16th century at the behest of the faithful who, in great numbers, witnessed the apparition of Our Lady in tears.

It has a mixtilinear crowning façade with a large entrance portal enriched by a tympanum and molded cornices. Almost near the roof there is a large square window and at the top left on six non-consecutive bricks we read IHS / Maria / D.R. ME / S.M.F. / MPF / 1637.

The interior is characterized by two naves arranged in a Latin cross whose intersection is surmounted by a fine dome, one of the largest in the province of Teramo, topped by an octagonal lantern on which rests a globe and a cross.

Also interesting is the high altar in Baroque style arranged in the apse and restored about thirty years ago. In the central aisle there are eight round-headed arched chapels, seven of which house an altar and the other a beautiful pipe organ from 1654. Next to the stone holy water is the plaque with the Cross of Indulgences by Pope Leone XIII.
