It is located in the main square of the village of Cologna Paese and shows an installation dating back to the seventeenth century consisting of a higher central body with gable roof and front gable and two lowered side bodies with a unique sloping roof opposed.

The façade is characterized by three portals of which the central one, the main one, has larger dimensions and is surmounted by a tympanum and a large paneling made with 150 castelli ceramic tiles arranged in 2001 and depicting San Nicola Vescovo, to whom consecrated the Church, and the Madonna who watch over the village and its inhabitants.

The interior is rich in architectural elements and devotional symbols and has a central nave much larger than the two side, separated by round arches resting on columns with a quadrangular base. Next to the acquasantiera are the diocesan symbol of the 2000 Jubilee and the plaque of the Cross of Indulgences granted in 1901 by Pope Leo XIII.
