It is in a slightly secluded position compared to the village of Fano a Corno, a small fraction of Isola del Gran Sasso. It has an ancient origin, since it was erected on the ruins of an old Fanum (temple) and from 1046 was managed by the Avellaniti of San Pier Damiani. The current building, however, is more recent and almost certainly is the result of a reconstruction of 1703 made necessary by the massive damage caused by the earthquake.

The roof is a gable and the façade has a mixtilinear crowning. The portal is surmounted by a large rectangular window and on the left rear wall stands a massive bell tower with a square base that houses three bells, the smallest of which is the oldest, since it dates back to the eleventh century. An important restoration of 1962 brought back to light a fresco of the Madonna del Rosario with the MDLXXX date.
