It is located near the sources of the Tordino River at about 1,000 meters above sea level and its original structure dates back to the late fifteenth century. Today's configuration is the result of a series of restorations carried out in the eighteenth and nineteenth centuries and, lastly, in 1988, following which it was elevated to the Sanctuary of the Madonna dei Monti della Laga. The roof is gabled, access is from one of the long sides and has an interesting bell tower with a large spire.

The church internally consists of a single nave that ends in the presbytery area that houses a wooden baroque altar of fine workmanship painted golden with spiral columns and angels, in the center of which is a statue of the Sacred Heart of Jesus. On the inside wall, near at the entrance you can see the remains of a fresco from the 16th century, while near the presbytery there are two Baroque altars.
