It is located between the houses distributed along the main road of the hamlet of Collecastino. It has a gabled roof and a simple facade with horizontal crowning on which is placed a brick bell tower that supports two bells.
It was almost certainly built in the mid-eighteenth century (perhaps in 1755). The portal that surrounds the entrance is completely brick, has side columns and shelves and the architrave has a curvilinear pattern, while two small windows with grilles open out laterally.
